Investigational Amylin Receptor Agonist for Weight Management Research

Eloralintide (LY3841136) is an investigational, long-acting injectable medication developed by Eli Lilly and Company and being studied for the treatment of obesity and related cardiometabolic conditions. It belongs to a class of compounds known as selective amylin receptor agonists and is designed for once-weekly administration.

Amylin is a hormone naturally released after meals that contributes to appetite regulation and feelings of fullness. Eloralintide is designed to preferentially activate the amylin 1 receptor (AMY1R), with the aim of reducing appetite and food intake and supporting body-weight reduction.

Eloralintide remains an investigational medicine and has not been established as an approved treatment for routine clinical use.

Mechanism of Action

Eloralintide is being investigated for its effects on amylin receptor signaling and the regulation of energy intake.

  • Selective Amylin Receptor Activation: Eloralintide has demonstrated preferential activity at the amylin 1 receptor (AMY1R) in preclinical studies. This receptor selectivity distinguishes it from some other amylin receptor agonists under investigation.

  • Appetite and Satiety Regulation: By activating amylin-related signaling pathways, eloralintide is designed to influence hunger, promote feelings of fullness, and reduce food intake.

  • Body-Weight Regulation: Preclinical research indicates that eloralintide can reduce food intake and body weight, primarily through reductions in fat mass in animal models. Its effects in humans have been evaluated in clinical trials.

Clinical Efficacy and Trial Results

Eloralintide has completed Phase 1 and Phase 2 clinical studies and is being evaluated in Phase 3 trials.

In a 48-week, randomized, placebo-controlled Phase 2 trial involving 263 adults with overweight or obesity and at least one weight-related complication, participants received different weekly doses of eloralintide or placebo.

Clinical Measure Reported Findings
Body-Weight Reduction Mean body-weight changes at week 48 ranged from approximately 9% to 20% reduction across the eloralintide treatment groups, depending on the dose and dosing regimen.
Placebo Group Mean body-weight change was approximately −0.4% at week 48.
Most Common Adverse Events Nausea and fatigue were among the most frequently reported adverse events. Their frequency varied across treatment groups and doses.

These results were published in The Lancet in 2025. They represent findings from a Phase 2 trial and do not establish the medicine's long-term effectiveness or safety across all populations.

Potential Therapeutic Applications

  • Obesity Research: Eloralintide is being studied as a potential pharmacological treatment for obesity and overweight in people with weight-related complications.

  • Appetite and Satiety Research: Its selective amylin receptor activity provides a basis for investigating the hormonal regulation of appetite and food intake.

  • Obesity-Related Conditions: Clinical development includes the investigation of conditions associated with obesity, including obstructive sleep apnea and knee osteoarthritis pain.

  • Combination Therapy Research: Eloralintide is also being studied in combination with tirzepatide, a dual GIP/GLP-1 receptor agonist. The combination, referred to as EloraTZP, is being evaluated separately and should not be confused with eloralintide monotherapy.

These are investigational applications. Eloralintide is not an established treatment for any of these conditions.

Current Development Status and Safety

As of October 2026, eloralintide remains an investigational medicine that has not been approved for routine clinical use. Eli Lilly is conducting Phase 3 clinical trials to evaluate its efficacy and safety in people with obesity, with or without related conditions.

In the Phase 2 trial, commonly reported adverse events included nausea and fatigue. The frequency of these effects varied by dose. The available findings do not establish the full long-term safety profile, and additional clinical data are required.

Eloralintide should not be presented as an approved weight-loss medicine or as a substitute for individualized medical assessment and evidence-based treatment.

Investigational compound — Clinical research findings are intended to support scientific evaluation and do not establish approval or suitability for human use outside authorized clinical trials.
