Investigational Injectable for Localized Fat Reduction

CBL-514 is an experimental, first-in-class small-molecule injectable drug developed by Caliway Biopharmaceuticals and currently undergoing clinical evaluation for the reduction of localized subcutaneous fat. Unlike weight-management approaches such as diet, exercise, or GLP-1 receptor agonist medications (including semaglutide and tirzepatide), which primarily influence appetite, food intake, and body weight, CBL-514 is designed to selectively induce apoptosis, or programmed cell death, in adipocytes (fat cells) at the injection site.

CBL-514 is administered by targeted subcutaneous injection. Its potential applications and long-term effects remain under clinical investigation.

Mechanism of Action

CBL-514 is designed to act locally on adipose tissue through the induction of adipocyte apoptosis.

  • Induction of Apoptosis: The compound is designed to activate programmed cell death in targeted adipocytes. The precise contribution of individual molecular pathways should be interpreted in accordance with the available published research.

  • Targeted Fat Reduction: By promoting adipocyte apoptosis, CBL-514 aims to reduce subcutaneous fat volume in the treated area. However, this does not establish that the cosmetic outcome will be permanent, as remaining fat cells can enlarge and body fat can accumulate again.

  • Selectivity and Tissue Safety: Clinical and preclinical findings reported to date suggest a potentially favorable local tolerability profile. Further research is required to establish the full safety profile and the risk of uncommon or long-term adverse effects.

Clinical Efficacy and Trial Results

CBL-514 has been evaluated in clinical trials for non-surgical subcutaneous fat reduction. Results presented at the 2026 Annual Meeting of the European Association for the Study of Diabetes (EASD) included the following findings:

Parameter Subcutaneous Fat Visceral Fat
Reported Reduction In a Phase 2 study, 69.6% of participants receiving CBL-514 achieved a reduction of at least 150 mL of subcutaneous fat in the treated area at the 8-week follow-up, compared with 0% in the placebo group. In an MRI-assessed subgroup of Phase 2 trial participants, average visceral adipose tissue volume decreased by 12.01% at the first follow-up and remained 10.45% below baseline at the later follow-up.
Placebo Group No participants achieved a reduction of at least 150 mL at the 8-week assessment. Average visceral fat volume increased by 5.68% at the first follow-up and by 11.76% at the later follow-up.

The visceral fat findings were obtained from a subgroup of participants with available MRI measurements. They should therefore be considered preliminary clinical evidence rather than definitive proof of efficacy across broader populations.

Potential Therapeutic Applications

  • Localized Fat Reduction: CBL-514 is being investigated as a non-surgical approach to reducing localized subcutaneous fat. It is not yet an established or approved alternative to liposuction.

  • Visceral Fat Research: Preliminary MRI data suggest a possible effect on visceral adipose tissue. Further studies are needed to confirm these findings and determine their clinical significance.

  • Combination with GLP-1 Receptor Agonists: Preclinical studies suggest that combining CBL-514 with tirzepatide may enhance weight loss and reduce weight regain after treatment discontinuation in animal models. A Phase 2 clinical trial is evaluating the combination; these benefits have not yet been established in humans.

  • Other Adipose Tissue Disorders: CBL-514 is being investigated for additional indications, including cellulite. A separate formulation, CBL-514D, is under development for Dercum's disease, a rare condition associated with painful lipomas. Investigational use does not imply regulatory approval for these conditions.

Current Development Status and Safety

As of October 2026, CBL-514 remains an investigational drug and has not been approved for routine clinical use or commercial marketing as a medicine. Its global Phase 3 development program includes the SUPREME-01 trial, which is evaluating its efficacy and safety for non-surgical abdominal subcutaneous fat reduction.

Clinical studies reported to date have generally described CBL-514 as well tolerated. Commonly reported adverse events have included temporary, mild-to-moderate injection-site reactions, such as pain, swelling, and redness. These findings do not establish the absence of serious risks or fully characterize long-term safety. Larger and longer-term studies are needed.

CBL-514 should not be presented as an approved treatment, a proven weight-loss medication, or a guaranteed permanent solution for localized fat reduction.
